Refund Policy


Once tokens for printing Emergency (NVDs) eDECs have been purchased, refunds will not be provided for unused tokens.
Cancellation or the withdrawing from the use of the Emergency (NVDs) eDEC program after purchase of Tokens does not provide a reason for requesting a refund.
You may request a refund if an incorrect or multiple charge has been paid as a result of a computer or banking error.
Requests for refunds must been submitted using the form below.
You must include by reference all relevant documents and outline all mitigating circumstances with this request for refund.
Request for refund must be submitted within 15 days of payment being made.
Completion and submission of this form does not constitute approval of a request for refund.
Applications for refunds are generally processed but not guaranteed to be processed within 21 days.
Approved funds may be subject to processing fees by both the financial institutions and LPA Administration.

I understand that no refund is guaranteed, that my request will be considered on the basis of information I have supplied and an administrative fee for processing for this application may be deducted from any possible refund.

I have included true and correct information in respect of this request for refund and should any information that I have supplied be found to be incorrect or falsified, I accept all applicable fees and charges related to the processing and recovery of costs associated with the original transaction and this request for refund application.
